How To Apply For CODA's Service
There are several ways to apply for a service with CODA. You can either contact the Service Manager or Coordinator directly, or be referred by another agency.
It is important to remember that there are a number of factors that determine who receives a service, how many hours, and why CODA has certain eligibility criteria.
Firstly, CODA provides a service to adults with a disability, between the ages of 18-65 years who are living at home with a primary carer. We also look at the support needs of the person, and those who have high support needs, or who may be at risk of inappropriate institutional care are given a high priority. This, however does not preclude others from a service, as we assess each person on individual basis, taking into account other factors, eg. Health, age of carers, other service provision involved etc.
Secondly, CODA has geographical boundaries, that determine service delivery. This geographical area extends from the Metropolitan Brisbane area (south of the river), south to Logan; east out to the Redlands shire; and as far West as Forest Lake.
Thirdly, due to the high demands for services, CODA does have a waiting list. Our waiting list is a NEEDS based one, not time based. Therefore, the person with the highest need at the time we are able to assist with a service, will be contacted. Because of this, it is important that you keep in regular contact with us, so that we may update your needs on an ongoing basis.
