DSOA Disability Support for Older Australians
DSOA funding is obtained through Department of Health and Aged Care and provides funded support to older people with disabilities who were not eligible for the National Disability Insurance Scheme (NDIS). The program assists individuals who were receiving specialist disability services from the Continuity of Support (CoS) Programme.

What supports does CODA offer under DSOA?
CODA is a registered provider under the DSOA program and offers a range of services to support individuals with disabilities. These services include:
1. Coordination of Supports: This involves helping individuals plan and coordinate their various support services, ensuring that their needs are being met effectively.
2. In-Home Support: Providing assistance and care services to individuals within their own homes. This might include help with daily tasks, personal care, and other forms of support.
3. Flexible Support: Offering adaptable and customizable support options based on the individual’s needs and preferences. This can include adjusting the type and level of support as required.
4. Community Access: Assisting individuals in participating in community activities and engaging with their local community. This can help promote social interaction and inclusion.
5. Supported Independent Living: Providing support to individuals who are living alone or with others and require assistance with certain aspects of daily life to maintain their independence.
6. Assistance with self-care and Daily Living: Assisting individuals with activities of daily living, such as bathing, dressing, meal preparation, and other essential tasks.
How do I request CODA to provide my DSOA funded supports?

Plan Management
CODA can provide Plan Management of your funds. We are all about tailoring our services to meet your needs. We actively work with you and your stakeholders to develop plans that empower, encourage and support you to maximise your potential and deliver outcomes. Your Support needs and expectations are identified during our initial process and monitored and reviewed ongoing.
We can help by working through a budget with you, allocating supports and services where it is most needed. Alternatively, you may have this all worked out already and we can further assist by:
- organising and sourcing providers and their payments
- processing of expense claims
- providing you with regular statements of your funding
- assisting to increase your skills in managing funding supports in your plan
CODA can assist in negotiating and coordinating the provision of support individualised for you. You can change the level of support for plan management or case management as needed as you develop skills. CODA supports the principles of choice, control and dignity.

Individualised Support
CODA provides one to one support to individuals with a disability to further life experiences and skill development in home and local community. Support may also be purely of a social or recreational nature.
We assist you to:
- Link with community groups e.g.
- Networking / friendship building
- Manage your household e.g.
- Assist you to do your banking / shopping / pay bills etc.
- Attend training or workshops
- Study or work/volunteer commitments
- Learn new skills or achieve chosen goals
We recognise your right to participate in your community and enjoy valued roles.
The Client Centered Plan / Care Plan is developed in consultation with you, your advocate and any other stakeholders who you wish to be involved in service delivery.
Client Service Coordinators roster skilled Disability Support Workers to work with you that hold minimum qualifications or relevant industry experience, hold a current first aid certificate and NDIS Worker Screening Clearance card.

Personal Care and In Home Support
CODA provides quality flexible in-home and community based support to individuals with a disability.
This support is delivered in a way determined by you, to best meet your needs.
Our trained friendly staff can assist you with:
- Toileting needs and routines
- Showering/ hygiene support
- Meal preparation and meal assistance
- Dressing support
- Light domestic support related to personal care tasks
- Washing clothing and linen
We understand that each person needs different types of support at different times, so we work with you to identify your specific wants and needs. We develop a support plan (care plan) that reflects what you want to happen and your support is tailored to how you have identified.
Client Service Coordinators roster skilled Disability Support Workers to work with you. All CODA’s Disability Support Workers hold minimum qualifications or relevant industry experience, hold a current first aid certificate and Disability Services positive notice card.

NDIS Support Coordination
CODA provides Support Coordination to help you understand and make the most of your NDIS plan. We work alongside you to build capacity, strengthen your networks, and ensure you have access to the services and supports that best meet your needs.
We assist you to:
- Understand your NDIS plan and funding
- Connect with service providers that align with your goals
- Navigate service agreements and bookings
- Coordinate supports and appointments
- Problem-solve challenges with services or funding
- Build the skills and confidence to manage your own supports over time
We recognise your right to exercise choice and control, participate fully in your community, and work towards the goals that matter most to you.
Your Support Coordination plan is developed in consultation with you, your advocate, and any other stakeholders you choose to involve.
Support Coordinators are experienced in the NDIS framework and work with you to ensure your supports are reasonable, effective, and contribute to positive outcomes.

Supported Holidays
CODA is committed to delivering a service that provides individuals with every opportunity to have meaningful and fulfilled lives.
CODA offers one to one supported holidays to destinations of the individuals choosing. We can also arrange small group holidays, if you wish to go away with a few friends!
Our aim is to ensure that we deliver a quality service to you, ensuring that you have a holiday experience to remember. Qualified Staff will support you in all aspects of your holiday, ranging from supporting you with booking requirements to tailoring the support you need while you’re away. This could be 24hr support or support when required.
